Have you had your hearing checked lately? Getting a hearing test is an important health move, since about two-thirds of people over age 60 have age-related hearing loss.1
Maybe you’ve noticed that your ears aren’t as sharp as they used to be, and you’re wondering if you need hearing aids. Or you already wear hearing aids, and you’d like to upgrade them. Even if you think your hearing is normal, a baseline hearing exam is a good idea.

Could you be one of them?
Click this link to get started. Click the blue button at the top of the page that says “Sign in or create an account.” Fill in your information or create an account to see your plan coverage.
Scroll down to the Zip Code box and enter your zip code. Then click the “Search” button.
Your search results will show a column of in-network hearing care professionals near you, plus a map so you can see where each provider is located.
The Premium Care providers will show up at the top of the list. (You’ll see a blue map pin with a starburst inside it next to their name.) These are top-rated hearing care professionals who serve members with the highest quality of care.
Don’t see a Premium Care provider that’s convenient for you? Check out the Recommended Providers, who are next on the list. These high-quality hearing care professionals are also a great option. (You’ll see a blue map pin with a plain star inside next to their name.)
Once you decide on a provider, click the blue “Request an appointment” button. It's directly below the provider’s name and address in your search results.
Finally, check the “Consent” box at the bottom of the page.
Once you submit your information, you’ll see a pop-up window that says, “Thank you for requesting an appointment.” The hearing team will verify your benefits. Then your chosen provider will reach out to you by email or phone to schedule a time and date for your hearing exam.
If you haven’t heard from the clinic in a few days, you can also give them a call directly. You’ll also receive a referral letter by mail or email once you’re matched to a practice.
